Allen Home Care Connection at Bridges is a department of Allen Hospital Home Health Services. Allen Home Care Connection provides on-site 24hour a day / 7 day a week home health services for Bridges residents with an individual service plan contract. Individual services plan contracts are developed by a Registered Nurse in collaboration with the resident and their family members, as applicable, to meet the individual residents health careneeds.

Services available for individual service plan contracts include, but are not limited to: nursing assessments, medication management, wound care management, diabetic management, lab draws, foot care, medication reminders, bathing / hygiene /dressing care, physical therapy, occupational therapy, speech therapy, rehab assistance, activity / meal escorts, reassurance checks, dietician consults andsocial work consults.
